Repeat Facility does not have an employee with manager certification in food protection issued from the Ohio Department of Health. Left ODH's application for managers certification in food protection forms. Correct by next inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed sticky residue on the inside of the slushie machine dispenser nozzles. PIC cleaned nozzles at the time of inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed a container of potato salad with an internal temperature of 44-45*F. PIC voluntarily discarded potato salad at the time of inspection.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed packages of sliced ham and turkey with a use by date of 10/8 and a pan of quinoa with a prep date of 10/2. PIC voluntarily discarded ham, turkey and quinoa at the time of inspection.

3717-1-03.5(C)(2) / Food labels - packaged food
Corrected During Inspection Label on a food packaged in-house did not contain all required information. Observed packages of cookies that were not labeled in the customer self-service area. PIC removed cookies from sale at the time of inspection.

Comments
Discussed inspection with the PIC. Discussed monitoring display cooler to ensure cold holding of 41*F or below. Facility cools and reheats soups in bulk approximately three times per week. Discussed cooling and reheating soups in individual portions or moving to a risk level four license on March 1, 2024. Submit specification sheets to RPH for review prior to purchasing new equipment. Emailed inspection report.
