Critical Corrected During Inspection PIC unable to demonstrate knowledge by having no critical violations. All critical violations corrected at time of inspection.

3717-1-02.4(C)(5) / PIC: duties - ensure employees are visibly observing foods upon receipt.
Critical Repeat Corrected During Inspection Person in charge did not ensure that employees observed foods as they were received. During discussion, PIC stated the facility does not take temperatures of received product. Ensure all foods have temperatures taken and are inspected upon receipt. Education provided at time of inspection.

3717-1-02.4(B)(2)(g) / PIC: Demonstration of Knowledge - Stating required temperatures and times for safe storage, holding, cooling, reheating of TCS food
Critical Corrected During Inspection PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating. During discussion, PIC could not stated proper hot holding temperature. Ensure hot holding temperatures remain 135F and above at all times. Education provided during inspection.

Critical Repeat Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed no signed employee illness agreements. PIC had signed at time of inspection.

3717-1-02.2 (B) / Hands and arms: cleaning procedure
Critical Corrected During Inspection Improper washing of hands and arms. Observed PIC wash hands and not use paper towel to turn off water, therefore recontaminating hands. EHSIT provided education and then PIC rewashed hands properly at time of inspection.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Repeat Corrected During Inspection Food equipment surfaces not cleaned at required frequency. Observed pizza bubble poker to not be cleaned since the day prior and observed with visible residue, therefore exceeding the 4 hour cleaning frequency requirement for food-contact surfaces not held at temperature. Facility is to increase cleaning frequency to every 4 hours. PIC had cleaned at time of inspection.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Corrected During Inspection Inadequate equipment available for cooling, heating or holding food. Observed cheese to be overflowing the load limit of the pan on the make line. To ensure proper holding and cooling, ensure all foods are held at or below the pans designated load limit. PIC corrected at time of inspection.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed no thermometer in the walk-in cooler or the pizza make line. Ensure a temperature measuring device is located in the warmest portion of both of these units. Correct by reinspection 1/11/2024.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Corrected During Inspection Food not protected from contamination. Observed personal items intermingled with items for sale, rendering the products for sale exposed to contamination. PIC relocated items to appropriate location at time of inspection.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed no beard net in use by food handling employee at time of inspection. Hair restraints can take the form of hair bands, bandanas, hats, beard/hair nets accompanying complete restraint of hair.Including servers who plate biscuits/salads/make speciality drinks etc. hair restraints are required as for these activities because they meet the definition of food handling employees. Correct by reinspection 1/11/2024.

Nonfood-contact surfaces constructed of unapproved materials. Observed sticker residue on the wall behind the pizza make line, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct by reinspection 1/11/2024.

Repeat Fixed equipment not properly sealed or spaced for cleaning. Observed no caulking on the front handwashing sink, rendering the area no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

Corrected During Inspection Nonfood-contact surfaces not easily cleanable. Observed cutter for parchment paper to not be smooth, durable, non-absorbent, or easily cleanable. PIC discontinued use of cutter at time of inspection.

Non-food contact surfaces of equipment are unclean. Observed inside ledge of make line reach in coolers to have visible residue. Clean by reinspection 1/11/2024.

Repeat Plumbing system not properly maintained or repaired. Observed reverse connection of hot and cold water at the and washing sink in the front area. Richland Public Health's Plumbing Department will be in contact. Correct by next standard inspection.

Plumbing fixtures unclean. Observed spray nozzle and facet at the 3 compartment sink to have visible residue. Clean by reinspection 1/11/2024.

Repeat No drain plugs in waste receptacle. Observed no drain plug in the dumpster. Contact provider in regards to obtaining required drain plug for dumpster to prohibit potential harborage. Obtain by next standard inspection.

Physical facilities not maintained in good repair. Observed holes in the walls throughout the facility, rendering them no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.
