3717-1-02.4(C)(5) / PIC: duties - ensure employees are visibly observing foods upon receipt.
Critical Corrected During Inspection Person in charge did not ensure that employees observed foods as they were received. Through discussion, it was verified that temperatures are not taking temperatures of products upon receipt. Ensure temperatures are being taken of TCS products to ensure no temperature abuse upon receipt. Education provided at time of inspection.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed employee illness agreement not updated for newest employees. Ensure employees are verifiability notified of their responsibility to report prior to beginning working with food. PIC had updated at time of inspection.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink not accessible. Observed pizza trays in the handwashing sink in the warewashing area. Ensure handwashing sinks are keep accessible and used for nothing except handwashing at all times. PIC removed pizza trays at time of inspection.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Corrected During Inspection Food equipment surfaces not cleaned at required frequency. Observed pizza oven poker with visible residue, upon discussion PIC stated that it is only cleaned once a night and has yet to be cleaned today (exceeded four hour time). Ensure all food contact utensils and surfaces are cleaned at least a four hour time frequency. PIC had sent to warewashing unit at time of inspection.

3717-1-03.2(M) / Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Observed several wet cloths stored on various surfaces throughout the facility. Ensure wet wiping cloths are stored in sanitizing solution when not in use. Correct by end of day 3/24/2023.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Fixed equipment not properly sealed or spaced for cleaning. Observed handwashing sink by the ovens to be coming away from the wall, rendering the surfaces no longer smooth, durable, and non-absorbent. Have sink fully sealed and installed by next standard inspection.

Critical Repeat Corrected During Inspection Food contact surfaces not easily cleanable. Observed scissors being used for food packages that are not easily separable. Ensure all scissors used for food packages are easily separable. PIC discontinued use of scissors until easily cleanable can be obtained.

Repeat Non-food contact surfaces of equipment are unclean. Observed dust accumulation on the to-go pizza green racks. Clean and maintain by 3/31/2023.

Plumbing system not properly maintained or repaired. Observed leaking spray nozzle from the warewashing area. Correct by next standard inspection.

Repeat Plumbing system not properly maintained or repaired. Observed facet of the handwashing sink by the ovens to have the water temperature connections reversed, as well as loosely connected. Correct by next standard inspection.

Repeat No drain plugs in waste receptacle. Observed no drain plugs in the provider dumpster. Contact provider in regards to obtaining required drain plug for dumpster to prohibit potential harborage. Obtain by next standard inspection.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Ventilation system not maintained. Observed ceiling vents over the food storage to have visible duct accumulation. Clean by next standard inspection.
