Repeat Unclean nonfood-contact surfaces. Shelves in reach in coolers have dried on residues and mildew. Clean before 04/15/19

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Floor tile in retail area missing and several ceiling tile were sagging. Please repair or replace before. 4/15/19.

Facility not maintained clean. Visible accumulation of dust and debris observed under and behind storage racks in retail area. Please clean before 4/15/19 and provide records of current pest control agreement with licensed applicator..

Unnecessary or nonfunctional items and /or litter on premises. Multiple non functioning reach in coolers observed in various states of disrepair. Please repair or replace by 4/15/19.

Most areas cleaned. Walk in cooler door and walk in cooler need cleaned.
Several areas of bare wood need painted.
