Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed no thermometer located in small milk cooler.

Critical Presence of live insects, rodents, and other pests. Observed flies in kitchen.
Action: Keep outer openings closed and provide fly control measures like Blue Lights.

Improper storage of food items. Observed box stored on floor in walk in cooler.
Action: Store boxes off of floor at least 6 inches. *Will recheck. Correct By: 02-Sep-2022

Equipment not approved by a recognized testing agency. Observed Panasonic Microwave that is non commercial.
Action: When unit no longer performs to standard, replace with Commercial Equipment.

Non-food contact surfaces of equipment are unclean. Observed sliding door rails Line Freezer with dirt and grime within the rails.
Action: Clean unit ASAP and clean on frequency to prevent build-up.

Facility not maintained clean. Observed floor fans and back external ventilation wall fan with heavy dust build up on each.

FSO did not have a personnel in charge per shift with the person in charge certification in food protection. Observation: During working shift serving food no personnel on staff was Basic Food Trained.
