Facility does not have an employee with manager certification in food protection. Discussed keeping any food safety training certificates on file in the facility for review. Correct by: 4/19/21.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Repeat Corrected During Inspection TCS foods not being hot held at the proper temperature. Observed cooked chicken on the stovetop at 131-133*F. PIC placed chicken in oven to reheat to 165*F at the time of inspection. Discussed proper hot holding temperature of 135*F or above. Digital food thermometer available to take internal temperatures.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed packaged bread stored on the floor of the walk-in freezer. PIC placed bread on the shelf at the time of inspection.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Corrected During Inspection Single-service and single-use articles not protected from contamination. Observed uncovered coffee filters stored on top of the coffee machine. PIC placed coffee filters back in the packaging to protect from contamination at the time of inspection.

3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Can opener blade dull or creating metal fragments. Observed metal shavings around the can opener blade. PIC had can opener cleaned at the time of inspection. Discussed replacing blade if metal shavings are observed around can opener.

3717-1-04.1(BB) / Warewashing machines - temperature measuring devices.
Critical Warewashing machine not equipped with thermometer as specified. Thermometer for dishwasher rinse cycle not operating properly at the time of inspection. Discussed contacting comopany for repair of rinse cycle gauge. Correct by: 4/19/21.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit for bleach observed at the time of inspection. Correct by: 4/19/21.

3717-1-05.4(H) / Toilet room receptacle - covered.
Repeat No covered receptacle in women's restroom. Correct by: 4/19/21.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed paint peeling on the wall above the stove. Correct by next inspection.

Comments
Conducted and discussed inspection with Janie, Nursing division, and Linda, kitchen staff. Provided RPH's updated employee illness reporting agreement form. Left a cooling foods safely handout. Re-inspection to occur on Monday, April 19, 2021. Emailed inspection report.
