3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed a knife with visible residue to be stored in a cup with clean utensils. PIC had all the utensils cleaned at time of inspection.

3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Cooking and/or baking equipment not cleaned when required. Observed the front microwave to have visible residue. Clean by reinspection 10/26/2023.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed containers of coney sauce and chicken in the sandwich cooler to both be date marked with a discard date of 10/17. PIC discarded at time of inspection.

3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects. Observed small flying insects surrounding the melting wells and the mop sink. Completely remove all evidence and/or contact a licensed pest control operator and retain the visit summary. Correct by reinspection 10/26/2023.

3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Repeat Can opener blade dull or creating metal fragments. Observed can opener blade to have metal shavings, allowing for physical contamination of foods. PIC discontinued use and plans to replace the blade today. Correct by reinspection 10/26/2023.

3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Corrected During Inspection Can opener blade dull or creating metal fragments. Observed the tip of the knife used at the sundae station to be chipped, allowing for physical contamination of foods. PIC discontinued use of the knife and replaced at time of inspection.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed leaking coming from under the cake freezer in the lobby. PIC stated plans to have repaired soon. Correct by next standard inspection.

Non-food contact surfaces of equipment are unclean. Observed the shelving in the freezer walk-in unit to have dried visible residue. Clean by reinspection 10/26/2023.

Non-food contact surfaces of equipment are unclean. Observed the outside of the oyster blenders to have visible residue. Clean by reinspection 10/25/2023.

Non-food contact surfaces of equipment are unclean.Observed the hot well of the banana dipper to have excessive visible residue accumulation. PIC started cleaning during inspection. Clean by reinspection 10/26/2023.

Plumbing system not properly maintained or repaired. Observed the drain under the 3 compartment sink to be leaking. PIC stated there is a plan in place to repair. Correct by next standard inspection.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed various places in the flooring that are chipped, rendering the areas no longer smooth, durable, non-absorbent, or easily cleanable. PIC stated there is a plan in place to repair the flooring. Correct by next standard inspection.
