3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Repeat Corrected During Inspection Observed employee eating, drinking, or using tobacco in non-designated area. Observed employee personal snacks in the food prep area and the clean dish area of the warewashing room. Personal snacks of candy and fudge pieces were voluntarily discarded by PIC and instruction was verbally given to employees.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result.
3717-1-02.4(C)(10) / Person in charge: duties - ensure employees are properly sanitizing equipment and utensils
Critical Corrected During Inspection The person in charge did not ensure that employees are properly sanitizing equipment or utensils. Food employee described the following process for cleaning a food contact utensil: wash, rinse, and spray on sanitizer. This is improper as all food-contact utensils must be sanitized by immersion. PIC re-educated employee at time of inspection, and will monitor process to ensure proper sanitization is being performed.
To prevent contamination, the person in charge shall ensure that employees are properly sanitizing cleaned multiuse equipment and utensils before they are reused.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed Jell-O cups in the walk-in cooler which did not have any protective cover. PIC placed a cover over product at time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.2(D) / Food storage containers - identified with common name of food.
Food storage containers are not properly labeled. Observed seasonings which bore no label of common name.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized. Correct By: 04-Jan-2018
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. Observed some badly dented/pitted equipment present in facility. Observed a badly pitted metal quarter pan, which PIC removed. Observed also two rolling food carts which were badly dented with rough, sharp corners which had been covered with duct tape.
Repair, remove, and/or replace equipment which is not smooth and easily cleanable. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Correct By: 18-Jan-2018
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. Observed the cappucino machine dispenser spouts to have a thick accumulation of dried powdered milk, coffee, and water residue. PIC removed for cleaning immediately and will ensure a proper cleaning schedule is followed going forward.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. Observed surfaces of equipment in need of greater cleaning frequency. Observed an insulation line on a boiler kettle to have a thick accumulation of grease. Observed dust build-up over the cook line area. Observed grease dripping above the grill line.
Nonfood-contact surfaces of equipment shall be kept clean. Correct By: 11-Jan-2018
3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Cooking and/or baking equipment is not cleaned at the required frequency. Observed the ovens in the facility to have a greasy film on the outter surfaces, and a build-up of black carbon residue and creosote on the inner surfaces.
Cooking and baking equipment and/or cavities and door seals of microwaves shall be cleaned at least every twenty-four hours. Correct By: 11-Jan-2018
3717-1-04.8(G) / Kitchenware and tableware - handling.
Corrected During Inspection Single-service or single-use articles, kitchenware, or tableware were not handled, displayed, or dispensed properly. Observed single-use eating utensils in two locations in kitchen having handles facing different directions.
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Repeat Corrected During Inspection The handwashing sink was not easily accessible. Observed access to the hand washing sink in the ware washing area to be blocked by a rolling supply rack. PIC relocated equipment at time of inspection.
To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Observed beneath all three (3) food preparation sinks: visible lengths of debris approximately 2 inches long extending between the discharge pipe and the drainage catch pipe. The drains must be cleaned regularly to preclude the accumulation of such discharge which may render the air gap ineffective to prevent back-tracking of bacteria or other deleterious substances up into the food sinks.

3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Correct By: 11-Jan-2018
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed the caulking seal behind the steam kettles to need replaced.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat The ventilation system was not being properly cleaned or was creating a public health hazard. Observed the air vents inside the pizza prep table cooler cabinet to have mildew growth on them.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. Correct By: 11-Jan-2018
Comments
Discussed inspection with Kitchen Manager Ricardo Cortez and Director of Food and Nutrition Services Gregory Brandes.

Facility is using an Aeroscout temperature control monitoring system for all cold holding equipment.

Observed some visible daylight at the rear receiving door.
-Ensure that doors and windows shut completely in order to effectively deny entry to pests.

Observed knives soaking in a food pan filled with water. Observed cooking utensils (spatulas) soaking in a sanitizer bucket. PIC removed both to the ware washing area, and provided verbal reminders that such a practice may not be used as a substitute for passing through the ware washer or a proper 3 step wash, rinse, and sanitize process.

Observed apples on trays being held inside cooler for later service. Food employee stated that they had not been washed.
-Ensure all washed, ready-to-eat fruits with an edible peel (apples) are always wrapped with protective cling film before leaving the kitchen area for service.

PIC has signed copies of employee illness reporting agreement on file. Document lists the five (5) required symptoms but only lists eleven (11) of the required thirteen (13) illness.
Provided RPH document which lists all required illnesses as well as symptoms.
-Keep signed employee agreement copies at facility and available for review at all times.
-Continue to have any new employees sign one as well.

PIC states any bodily fluids spills/incidents are handled through Hospital Facilities. Sanitarian will follow-up with EVS Supervisor to ensure proper protocol is met.
