3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed quart containers stored clean under the coffee maker area to have visible accumulation on the food contact portion of the container. PIC sent containers to warewashing unit at time of inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Repeat Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed sanitizer buckets in the kitchen to be over 200ppm. Ensure chlorine solutions are at a concentration of 50-100ppm. PIC had buckets remade at time of inspection.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed scissors that were not easily separable for cleaning being used to open food packages. If scissors are to be used for food packages, ensure they are easily separable. PIC discontinued use of these scissors until a screw driver can be obtained, or new scissors obtained.

3717-1-06.4(K) / Controlling pests.
Critical Presence of rodents. Observed rodent droppings in the drawers of the east dining hall cabinetry, dry storage and tray storage areas, and the back corner of the kitchen by the ovens. Clean all evidence, monitor the area, and contact a licensed pest control provider by reinspection 3/9/2023.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or single-use articles, kitchenware, or tableware not handled, displayed, or dispensed properly. Observed handles in the drawer below the microwave to be in disarray. Ensure all handles are facing the same direction to avoid cross contamination via hands. Correct by reinspection 3/9/2023.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed can opener blade with chips rendering the surface no longer smooth, durable, nonabsorbent, and easily cleanable. Ensure can opener blade is maintained to avoid metal shaving/chip contamination. PIC discontinued use of can opener at time of inspection until new blade can be obtained. Have new blade by reinspection 3/9/2023.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment not approved by a recognized testing agency. Observed noncommercial grade kitchen aid mixer being used in the facility. When unit no longer functions as designed, commercial grade equipment, tested and approved by recognized testing agency is to be obtained.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed ice build-up in the freezer rendering the surfaces no longer smooth, durable, non-absorbent, and easily cleanable. Clean by reinspection 3/9/2023.

3717-1-04.2(H)(1) / Temperature measuring devices - manual warewashing.
Corrected During Inspection Manual warewashing temperature measuring device not readily accessible. Observed no thermometer for the warewashing area. Ensure warewashing area has a thermometer separate from the food thermometers. PIC had new thermometer labeled and placed in warewashing area at time of inspection.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available. Observed all chorine test strips to be expired from 2017 and 2018. PIC stated she had just purchased test strips recently. Ensure test strips are within date upon purchase. Obtain valid test strips by reinspection 3/9/2023.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed wire shelving in the walk-in to have visible residue. Clean by reinspection 3/9/2023.

No covered receptacle in women's rest-room. Observed no covered trash can in the employee rest-room. Obtain covered receptacle by reinspection 3/9/2023.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed coving in the kitchen hallway, and under the bulk ice machine to be missing. PIC stated there is a plan in place to replace the coving. Correct by next standard inspection.

Repeat Physical facilities not maintained in good repair. Observed tile broken in corner of kitchen near mixers. Repair by next standard inspection.

Physical facilities not maintained in good repair. Observed ceiling tile above the spray nozzle to be missing. PIC stated there is a plan in place to replace the tile. Correct by next standard inspection.

Comments
Conducted and reviewed inspection with PIC.
Observed smell of natural gas originating from the storage room. Maintenance was notified and locating the leak. This observation was referred to Mansfield City Building and Codes Department.
Juice from concentrate is approved for use at the facility.
Facility level 4 activities include cook/cool/reheat of pork roast (CCP).
Reinspection to occur on 3/9/2023.

I - P - Employee Health: The operation had an employee health policy on file.
X - P - Chemical: Toxic materials are properly identified and stored.
