3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Repeat Presence of dead pests or insects. Observed dead insects in the light shield throughout the facility. Remove by next standard inspection. REINSPECTION: PIC stated plans to remove this week. Correct by next standard inspection.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed various portions of the floor throughout the facility with chipped or cracked tiles, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC stated plans for replacement later this year. Correct by next standard inspection.

3717-1-02.4(B)(1)/PIC: Demonstration of Knowledge - No Critical Violations
PIC unable to demonstrate knowledge by having no critical violations.
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed floors in the freezer to have visible residue and ice. PIC removed ice at time of inspection, clean floors by reinspection 3/26/2024.
3717-1-05.1(E)/Backflow prevention device - design standard.
Improper backflow prevention device installed on a water supply system. Observed no backflow on the spray nozzle near the custard machine. Ensure either a ASSE 1011 hose bib vacuum breaker is installed properly. PIC stated one has been ordered. Correct by reinspection 3/26/2024.
