3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection TCS foods were not being held at the proper temperature. Observed cold holding unit GMCW holding temp. of 45.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. Corrective Action: Milk and food items were removed at time of inspection.
3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment is not approved by a recognized testing agency. Observed Magic Chef Microwave not of commercial grade.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Action: When unit no longer performs to standard replace with commercial NSF equipment.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed gasket Maxx Cold Gasket is separated on door.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. Observed can opener blade dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correction: Employee cleaned blade at time of inspection.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. Ceiling tile missing in various locations in kitchen.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed light bulbs or heat lamp that were not properly shielded or coated where required. Observed two fluorescent light bulbs that were non shielded.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

Observed no handwashing sinks where required. Observed no designated hand wash station located in kitchen area.
Handwashing sinks shall be provided as specified under paragraph (G) of 3717-1-05.1 of this code.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris. Observed dirt and dust on floor walls and floor fan shield.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

Repeat Observed a build-up of dirt and debris. Observed basement area to have clutter and debri throughout..
