3717-1-02.4(B)(1) / PIC: Demonstration of Knowledge - No Critical Violations
Critical Corrected During Inspection PIC unable to demonstrate knowledge by having no critical violations. All critical violations corrected at time of inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed chlorine warewashing to have a concentration of 0 ppm. Ensure machine is capable of sanitizing with chlorine at a concentration between 50-100ppm. PIC stated she is putting another work order in for it today. Facility will be utilizing 3 compartment sink until dishwasher can reliably sanitize at proper concentration.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Repeat Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed a sanitizer bucket sitting on the 3 compartment sink to have a chlorine concentration above 200ppm. Ensure all chlorine solutions are made to a concentration between 50-100pm. PIC remade at time of inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed half gallon of milk in the walk-in cooler to have no datemark and PIC was unable to verify when the milk was opened. PIC voluntarily discarded the milk at time of inspection.

3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Presence of dead insects. Observed dead ants in the dry storage room around the baseboards. Clean by end of day 11/8/2023.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Corrected During Inspection Presence of live rodents. Observed rodent droppings on the floor under the mixers in the right hand side of the kitchen. Facility is on pest management plan and provided pest control logs at tie of inspection. PIC cleaned and sanitized the area at time of inspection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Corrected During Inspection Equipment components are not intact, tight or properly adjusted. Observed tip of white handled knife to have the tip chipped off, allowing for the potential of metal contamination. PIC voluntarily discarded the knife at time of inspection.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Corrected During Inspection Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard being used in the walk-in for onion bags to sit on, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC had removed at time of inspection.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed mechanical warewashing machine to be leaking sanitizer from the dispenser, unabling the unit to dispense the correct amount of chlorine to effective sanitize the dishes. Additionally observed the manifold thermometer for the rinse cycle to be in disrepair. PIC stated that a technician has been out once for the issue, it the issue persist. PIC stated she is putting a new work order in for the unit. Correct before putting the machine back into operation.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed kitchen aid mixer in the kitchen that is not commercial grade. PIC stated they do not use it. Remove from facility by next standard inspection.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed two pairs of scissors being used that are not easily separable, rendering the surface no longer smooth, durable, non-absorbent, or easily cleanable. PIC discontinued use of scissors until separable ones can be obtained.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Observed the prep sink pipes and catch drain to have visible residue. Clean by 11/15/2023.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed drywall covering a hole in the ceiling above the warewashing spray nozzle, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct/seal by next standard inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed floor under the prep sink to have visible residue. Clean by 11/15/2023.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Corrected During Inspection Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed hole in the coving in the kitchen behind the mixers allowing for a hole in the wall to be seen. PIC had the coving replaced at time of inspection.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed hole/crack in the ceiling near the light above the 3 compartment sink. Correct by next standard inspection.

VI - TCS Food: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. (See item #23)
IV - P - Demonstration of Knowledge: The person in charge is Certified in Food Protection.
III - P - Preventing Contamination by Hands: Observed food employee washing hands when required.
