Violations: A summary of the violations found during the inspection are listed below.
3717-1-06.4(K) / Controlling pests.
Critical Repeat Presence of live insects, rodents, and other pests. At time of inspection, observed multiple flying insects in various areas throughout facility (bakery, deli, mop sink near grocery receiving area). PIC states facility does have a monthly maintenance program with a licensed pest control company and observed last pest control report to indicate no findings from 8/1/23. Discussed reaching out to pest control company regarding treatment for observed pests currently. At 8/30/23 reinspection, observed improvements in deli\bakery area as minimal pests observed. Observed large amounts of flying insects in produce area near produce walk in cooler, as well as near mop sink in SFS area. PIC states pest control came out for a consultation but did not do any treatment at that time, instead discussed areas that needed repaired and provided some cleaning recommendations. PIC states they did repair a drain area in the produce room and are suppose to do using a drain cleaner daily. Discussed ensuring facility is thoroughly cleaning all recommended areas, as well as using the cleaner as instructed to aid in ridding of pests. Also discussed importance of continuing to follow up with pest control company regarding all possible options, as well as continuing to make efforts to completely rid of all pests observed. Facility provided report to show consultation visit from pest control company at time of inspection. Facility to email specialist at Richland Public Health with any continued progress or reports from pest control company. Reinspection to occur on 8/13/23. -Correct by 8/13/23

Nonfood-contact surfaces not easily cleanable. Observed stock carts in walk in meat cooler to contain rust and no longer easily cleanable. Resurface or replace carts. -Correct by next standard inspection.

Nonfood-contact surfaces not easily cleanable. Observed rust and peeling\chipping finish on metal shelving in produce cooler. Resurface or replace metal shelving. -Correct by next standard inspection.

Nonfood-contact surfaces not easily cleanable. Observed handle on microwave in deli area to be broken, rendering surface no longer easily cleanable. Repair or replace handle. -Correct by next standard inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed visible residue build up on speed racks and baking sheets in bakery area. At time of inspection, observed some progress as some racks and baking sheets have been replaced and\or thoroughly cleaned. Thoroughly clean remaining racks to remove visible carbon/burnt build up by next standard inspection.

3717-1-04.5(C)/Cooking and baking equipment - cleaning frequency
Cooking and/or baking equipment not cleaned when required. Observed visible dried residue on interior surfaces of microwave in deli area. Thoroughly clean interior surfaces of microwave. At reinspection, continued to observe dried residue on upper interior surface. -Correct by 8-30-23
3717-1-06.4(A)/Repairing.
Physical facilities not maintained in good repair. Observed visible condensation build up on multiple retail coolers, cooler\freezers doors, and water from condensation on floors throughout the facility. PIC states a work order has been placed regarding the issue. Discussed continuing to ensure condensation is cleaned up and food is not stored directly under drippage, as well as monitoring coolers closely until issue if resolved. At reinspection, observed less condensation in retail coolers, however, issue still persist some. -Correct by next standard inspection.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Facility not maintained clean. Observed wall near 3 comp area and utility area in deli to contain visible residue. Thoroughly clean walls. At time of reinspection, continue to observe visible residue on wall at 3 compartment sink in deli area. -Correct by 8-30-23.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Facility not maintained clean. Observed fan guards on condensers in deli walk in cooler and walk in produce cooler to contain visible dust accumulation and in need of greater cleaning. Thoroughly clean fan guards. At reinspection, observed fan guards in deli walk in to be correct, however, fan guards in meat walk in cooler still in need of greater cleaning. -Correct by 8-30-23.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Facility not maintained clean. Observed areas of the floors in need of greater cleaning. Thoroughly clean floors with special attention to retail aisles for chips, flour\sugar area, dairy walk in cooler, deli walk in cooler, and areas where standing water is observed from condensation drippage. PIC did have areas of the floors near the chips and flour\sugar retail areas cleaned\corrected at time of inspection. At time of reinspection, observed floors in aisles and dairy walk in to be correct, however, deli walk in cooler remains in need of greater cleaning. Correct by 8/30/23.
Comments
Inspection conducted and reviewed with coach\PIC and manager. Discussed observation of water sitting in floor drain in deli area. PIC states the basket inside the drain may need cleaned out. At time of inspection, discussed significance of uncorrected critical violations and that if significant progress is not made, a reinspection with the EH supervisor from RPH will be required, as well as a potential file review to determine if facility will enter into enforcement procedure.
