3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) did not have their hair effectively restrained. Observed food employee not wearing hair restraint.
To prevent contamination, food employees shall effectively restrain or cover hair to keep their hair from contacting exposed food, clean equipment, utensils or linens or unwrapped single-service or single-use articles.
3717-1-03.2(A)(2) / Preventing contamination from hands - bare hand contact
Critical Corrected During Inspection Observed food employee touching ready-to-eat food with bare hands. Observed employee placing RTE doughnuts on tray with bare hands.
To prevent contamination, except when washing raw fruits and vegetables, food employees may not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils. Al doughnuts touched by employee shall be discarded.
3717-1-03.5(C)(1) / Food labels - packaged food
Corrected During Inspection A food packaged in the facility did not contain a label. Responsibility Statement missing from tart dried cherries in ziploc bags.
Foods shall be labeled as specified in this rule. Place responsibility statement including name, city, state and zip code.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Equipment and/or utensils contain improper materials. Non food grade container storing flour.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed mutliple homeuse equipment throughout facility: Avanti Microwave, Kenmore freezer/refrigerator combination, kitchen aid mixer (handheld & stand), Keurig Coffee Pot (appears to be new), multiple crockpots, and Roaster pan.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. When and if these pieces of equipment no longer perform to satisfactory, replace with commercial grade equipment. Any new piece of equipment entering facility needs to be commercial grade and approved in writing by RPH prior to implementation.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Observed accumulation of soil residue on nonfood-contact surfaces. Observed last two fan guards to walk in condensor unit coolers to be covered with white fuzzy mildew. Observed hood filters to back room doughnut fryer to contain grease and dust.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean fan guards and hood filters.

The physical facilities are not being maintained in good repair. Observed 1st condensor unit inside walk in cooler to be dripping onto the ground.
