Critical Corrected During Inspection Handwashing sink not accessible. Prep table was blocking the hand sink by the drive thru. PIC moved at time of inspection.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Repeat Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Observed a pickle in the hand sink up front. PIC cleaned at time of inspection.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed the top rack of baked cookies and thawing bread uncovered. Clean wax paper was used to cover the top pans of bread and cookies at time of inspection.

Critical Repeat Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Sanitizer solution in three compartment sink was below 200 ppm. PIC brought solution to 200 ppm.

Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed the top layer of salami in the make line at 45*F. PIC voluntarily discarded out of temperature product. Discussed fill levels on containers in the make line.

Critical Corrected During Inspection Working containers of poisonous or toxic materials not properly labeled. Observed a spray bottle of water that was not labeled.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. No thermometer available in the drink cooler containing milk. Correct by: 11/21/19.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed buckets of pickles being stored on the floor. Food items shall be stored at least six inches above the floor. Correct by: 11/21/19.

Repeat Fixed equipment not properly sealed or spaced for cleaning. Observed the prep sink and the hand sink by the drive thru not properly sealed to the wall. Correct by next inspection.

Facility not maintained clean. Observed standing water on the floor of the walk-in cooler. Observed a build-up of ice on the floor of the walk-in freezer. Correct by: 11/21/19.

Repeat Physical facilities not maintained in good repair. Wallpaper by drive thru is peeling off the wall. Correct by next inspection.

Unnecessary or nonfunctional items and /or litter on premises. Observed bags filled with garbage on the ground behind and on the sides of the dumpster. Correct by:11/21/19.

Comments
Discussed inspection with manager Toni Ernest. Observed a ceiling tile missing above the walk-in cooler. PIC stated tile had been removed for cooler repair. Discussed replacing tile after unit is repaired. Observed a box fan in the kitchen with an accumulation of dust. PIC stated fan is used in the summer months. Discussed cleaning fan or removing it from the premises. Discussed deep cleaning of facility (walls, floors, counters/cabinets) before re-inspection. Re-inspection to occur on November 21, 2019. Emailed report.
