3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed area between fryers and wall, as well as between fryers and side of cabinets to contain residue. Correct by operation.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Observed cabinets to have residue. Ensure routine cleaning of non-food contact surfaces of mobile unit. Correct before operation.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed caulking along prep sink to be in disrepair. Repair/place to ensure smooth cleanable surface. Correct by operation.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed seal along counter backsplash to left of fryers to be in disrepair. Repair or replace to ensure smooth cleanable surface. Correct by operation.

3701-21-02(H) / FSO must display icense; Display of Mobile FSO information
FSO license not displayed; improper display of mobile FSO information. Observed mobile unit to not have name, city of origin, area code, and phone number posted on outside. PIC was able show stickers with proper info intended to be applied to outside of unit. Correct before operation.

Comments
Inspection conducted with Jen Lacey, EHSIT & Matt, owner/PIC. Ensure mobile unit contains for references for bodily fluid cleanup procedures, employee health policy, and choking poster. At time of inspection, coolers and freezers were not on site and unable to be verified for proper working order. At this time, unit is not currently approved for cold holding products. PIC states unit will only be selling french fries and additional coolers and freezes may potentially be obtained for further review. Observed unapproved backflow device at time of inspection. PIC was able to obtain ASSE 1024 backflow device and brought into office same day for verification/approval. At time of inspection, mobile unit was using well water. PIC states unit will be using only water provided at events when in operation. PIC ensures food grade hoses will be sanitized prior to operation.
Water temp of 126*F observed. Unit uses bleach as sanitizer and observed proper test strips on site. Probe thermometer on sight.
