3717-1-02.4(C)(5) / PIC: duties - ensure employees are visibly observing foods upon receipt.
Critical Corrected During Inspection Person in charge did not ensure that employees observed foods as they were received. Through discussion, PIC verified that temperatures are not being taken on cold items once delivered. Ensure facility has access to thermometers in order to taking temperatures of received cold items to ensure they are 41F of below. Education provided at time of inspection.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection No written procedures for responding to vomiting or diarrheal events. Observed no written fecal/vomit incident clean-up instructions at the facility. Provided instructions at time of inspection.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Observed no ambient thermometer for the dairy cooler at time of inspection. Obtain and install thermometer by 3/20/2023.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed receiving door in the storage area to have an inch gap on the left side of the door, allowing light and debris to come through. Ensure all outer openings are completely sealed to prohibit the potential for pest harborage. PIC stated there has been in order in to fix the door, repair by next standard inspection.

Repeat Non-food contact surfaces of equipment are unclean. Observed visible residue on the bottle shelving of the frozen food freezer and the dairy cooler. Clean by 3/20/2023.

Non-food contact surfaces of equipment are unclean. Observed various shelving units in the chip and cracker aisle with visible dust accumulation. Clean by 3/20/2023.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Observed mop sink with visible residue accumulation. Clean by 3/10/2023.

3717-1-05.4(N) / Covering receptacles.
Waste receptacles not covered properly. Observed dumpster doors open at time of inspection. PIC stated the waste receptacle providers placed dumpster too close to the building and facility is unable to close the dumpster doors. Contact provider to move dumpster to location where doors can be opened and closed as intended. Ensure dumpster door remains closed after each use to prohibit the potential for pest harborage. Close by 3/20/2023.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the floors under the chip rack end cap in the retail area, and the back storage area floor to wall junction to have visible residue accumulation. Clean by 3/20/2023.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed tiles in the employee rest-room with chips, cracks, pits and staining, rendering the surfaces no longer smooth, durable, non-absorbent, and easily cleanable. PIC stated work order was in for the repair but is unsure on the status. Repair or replace by next standard inspection.
