3717-1-02.4(A)(2) / PIC - Manager Certification
Facility does not have an employee with manager certification in food protection. Observed serv safe level 2 manager certificates, however, no Ohio Department of Health (ODH) certificate observed. Correct by next standard inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed sanitizer container empty at dishwashing machine and concentration at 0 ppm. PIC had employee refill sanitizer and dishwasher observed to be at proper strength during inspection.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers not properly labeled. Observed multiple squeeze bottles of dressings/sauces and shaker containers not labeled. Ensure working food storage containers are labeled with common name. Correct by 2-8-23.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed employees in warewashing area not wearing hair restraints. Also observed food employee preparing food with facial hair to not have hair restraint for facial hair. Correct today.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection In-use utensils improperly stored. Observed handle on scoop used for lettuce in salad prep cooler to be in contact with lettuce. Employee relocated scoop during inspection.

3717-1-04.8(G)(2) / Kitchenware and tableware - handling.
Corrected During Inspection Knives, forks and spoons that are not prewrapped improperly presented. Observed forks in clean storage in servers area in kitchen to be stored with lip contact portion up instead of handles up to prevent contamination. PIC removed forks and placed in warewashing area at time of inspection.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed small cutting board along cook line to contain staining and heavy scoring. Replace or resurface by next standard inspection.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment not properly sealed or spaced for cleaning. Observed seal along wall juncture on mop sink o be missing and peeling. Repair seal by next standard inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed plastic coating on handle of tongs to be cracked/broken. Also observed multiple plastic containers, top edges of bulk ice bucket, and plastic lids throughout facility to be broken on edges, rendering them no longer easily cleanable. Repair or replace by next standard inspection. PIC did discontinue use of ice bucket at time of inspection.

Non-food contact surfaces of equipment are unclean. Observed carbon/brunt food residue and build up along edges of grill at cook line. Clean by 2-8-23.

Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed back sides of lids in clean storage above prep sink to contain visible residue from splash. PIC had employee remove lids and place in warewashing area to be cleaned at time of inspection.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Multiple hand sinks not working at time of inspection. Work order in place. Correct by next standard inspection.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Insufficient air gap between the flood rim and the water supply inlet. Observed drain from ice storage bin at fountain machine in bar area not air gapped. PIC secured drain to allow for sufficient air gap at time of inspection.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed tile on floor along cook line missing. Repair by next standard inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed multiple areas of flooring throughout facility in need of greater cleaning. Special attention to floors under equipment along cook line and floor under storage rack for bag in box fountain syrups. Clean by 2-9-23.

3701-21-22 / Food choking standards
Corrected During Inspection Food choking standards not readily accessible in FSO. NO first aid for choking sign observed. Provided ODH's first aid for choking handout at time of inspection.
