Food-contact surfaces and utensils not sanitized properly. Observed dish machine with low sanitizer amount being added under 50 PPM.
Action: Correct ASAP.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Observed dust residue on top of Dish Wash Machine.
Action: Clean machine on frequent basis to prevent future build-up.
3717-1-05.4(H) / Toilet room receptacle - covered.
No covered receptacle in women's restroom. Observed no covered trash can in women's restroom.
Action: Provide covered trash can ASAP.

Grease trap is serviced routinely, stated PIC.
-Discussed requirement for clean-up kit.
-Chemical test papers are outdated ask PIC to replace Test Papers ASAP.
-Thermometer is available and properly calibrated.
-Level 4 activity includes:: Facility cooks, cools, and reheats.
-PIC will notify Health Dept. when dish machine is fixed.
