Critical Repeat Employees are not informed in a verifiable manner of their responsibility to report information about their health. PIC unable to locate employee illness reporting agreements at time of inspection. Correct by 3-23-23.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed internal temperature of half & half in automatic dispenser to be 47*F. PIC discarded product at time of inspection. Ensure TCS foods are held at or below 41*F.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed bags of half and half, whole milk, and iced coffee not date marked. PIC unsure when bags were opened and discarded at time of inspection. PIC replaced with properly dated bags.

Critical Repeat Corrected During Inspection Improper display of poisonous or toxic materials offered for retail sale. Observed lighters stored on counter above packaged candy at register, as well as lighter fluid stored above bottles beverages in back storage room. PIC relocated to separated area at time of inspection.

Repeat Corrected During Inspection Improper storage of food items. Observed box of chicken breast bites stored on floor in walk in freezer. PIC moved to a shelf, off the floor during inspection. Ensure food is stored at least 6 inches off the ground.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Non-durable equipment observed. Observed automatic creamer and milk dispense to have ambient temperature of 45*F. PIC placed dispenser out of order during inspection and state she will have it repaired or replaced. -Correct by 3-22-23.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed door gasket on door of walk in beer cooler torn/broken, as well as kick plate on door to be separating from door. -Repair/replace by next standard inspection.

Cleaning agents and/or sanitizers not available in the RFE. At time of inspection, PIC states they do not have sanitizer. Obtain sanitizer by 3-22-23.

Repeat No sanitizer test kit available. At time of inspection, PIC unable to locate sanitizer test strips. Obtain test strips by 3-22-23.

Non-food contact surfaces of equipment are unclean. Observed metal shelving and plastic sliders in beverage retail coolers to contain visible residue. -Clean by 3-22-23.

Waste receptacles not covered properly. Observed side door on trash dumpster open and not in use at time of inspection. Ensure doors remained closed when not in use to deter potential pest horborage. -Correct today.

Facility not maintained clean. Observed floor underneath shelving and along outer perimeter of walk in cooler and freezer to contain visible build up and trash. -Clean by 3-22-23.

Inspection conducted with manager/PIC. Discussed retaining daily cleaning maintenance logs for F'real machine and provided copy of cleaning instruction handouts and logs at time of inspection. Inspection report emailed.
