3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed Magic chef microwave and home use toaster non commercial.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Action: When units no longer perform to standard replace with commercial NSF equipment only.
3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Observed re-use of single-service or single-use articles. Observed slaw in cottage cheese container.
Single-service and single-use articles may not be reused.
3717-1-05.1(G) / Handwashing sinks-Number.
Repeat Observed inadequate number of handwashing sinks. Observed no designated hand sink in facility.
To ensure frequent and proper handwashing, at least one handwashing sink, a number of handwashing sinks necessary for their convenient use by employees in areas specified under paragraph (L) of this rule, and not fewer than the number of handwashing sinks required by the Ohio building code shall be provided. Action: Provide hand sink that is not used for any other purpose.
3717-1-05.1(I) / Service sink - number.
Observed no service sink or curbed cleaning facility as required. Observed no service sink in place to dump mop water.
At least one service sink or one curbed cleaning facility equipped with a floor drain shall be provided and conveniently located as specified in this rule. Action: Referred to plumbing department.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed ceiling tile missing in kitchen area.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Action: Provide ceiling tile to prevent dust and provide cleanable surface.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. Observed fluorescent light bulbs without protective shielding in place.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. Observed floor fan shield that has dust on units fan and shield.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean unit prior to using.

-Discussed level 3 and bulk re-heating facility will only reheat one food item one time per week, reheat in individual portions for the rest or else move up to level 4
