3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. -Observed a build-up of old milk residue on the food contact surface of the espresso machine. PIC properly cleaned and sanitized. -Observed brown and orange residue in the slushy machine nozzles. PIC stated this machine will not be used again until the fall and stated she will properly clean and sanitize.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. -Observed and opened gallon of milk with no proper date mark. PIC voluntarily discarded one gallon of milk.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Corrected During Inspection Inadequate equipment available for cooling, heating or holding food. -Observed a Beverage Air cooler with cheese being cold held inside. Equipment label states "For the storage and/or display of prepackaged or bottled beverages only." PIC moved cheese to front display cooler.

3717-1-03.5(C)(1) / Food labels - packaged food
A food packaged in the facility not labeled. -Observed pre-portioned containers of chili in the customer-self service freezer to be missing manufacturer and quantity on the label. PIC states she will place the manufacturer label and quantity on each individual food container.

3717-1-03.2(M) / Wiping cloths - use limitation.
Corrected During Inspection Improper use and/or maintenance of wiping cloths. -Observed wiping clothes throughout the facility being used underneath equipment/utensils, over boxes of products and as covers for equipment. PIC properly removed all dry rags that were not meant for wiping food spills.

In-use utensils improperly stored. -Observed scoop handles in sugar and donut mix to be in direct contact with the food product.

[51] Improper storage of cleaned equipment, utensils, and laundered linens. -Observed utensils stored in a bin to be facing different directions.

Equipment not approved by a recognized testing agency. -Observed multiple household appliances throughout facility: white Kenmore refrigerator/freezer unit, Kitchen Aid mixer, Oster toaster, Haier mini fridge, and multiple crock-pots and roaster.

Non-food contact surfaces of equipment are unclean. -Observed the gasket of the Kenmore refrigerator/freezer unit to contain food residue. -Observed sticky food spillage in the white chest freezer in the back area.

Repeat Unclean nonfood-contact surfaces. -Observed multiple lids to dry food containers to contain flour/food debris. -Observed condenser unit and ceiling to walk-in cooler to contain an accumulation of dust/mildew.
