Conducted inspection with PIC Cindy Townley; reviewed inspection findings with PIC. Risk Level 3 activities include Cook/Serve. According to PIC, leftovers discarded. Menu consisted of Pizza Sub Sandwich (cook,hot hold), green beans (cook,hot hold), prepackaged slushy, and pasteurized milk. Dishwasher is in disrepair. Three compartment sink in use for utensils, pots and pans.
