PIC unable to locate signed copies of employee health policies at the time of inspection. Provided RPH's employee illness reporting agreement at the time of inspection.

Corrected During Inspection No written procedures for responding to vomiting or diarrheal events. Provided ODH's guidance for clean-up of vomit/fecal accidents.

No soap at the back handwashing sink.

No towels or drying device at the handwashing sink(s). No paper towels available at the back hand washing sink.

Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Sanitizer solution at 100 ppm in the three compartment sink at the time of inspection. PIC drained and refilled solution to a concentration of 200 ppm at the time of inspection.

Fixed equipment not properly sealed or spaced for cleaning. Observed the hand sink by the oven not properly sealed to the wall. Correct by next inspection.

Equipment components are not intact, tight or properly adjusted. Observed one of the pull out drawers missing on the make table cooler. PIC stated drawer is scheduled for repair. Correct by next inspection.

Repeat Manual warewashing temperature measuring device not readily accessible. Discussed obtaining a thermometer to keep on the shelf above the three compartment sink. Correct by next inspection.

Critical Corrected During Inspection Manual wash solution not being maintained at 110˚ F or above. Observed the wash water at 104*F at the time of inspection. PIC added hot water to bring solution to 110*F during inspection.

Repeat Plumbing system not properly maintained or repaired. Observed the faucet handle loose at the hand sink by the oven. Correct by next inspection.
