Critical PIC unable to demonstrate knowledge by having no critical violations. Observation: Facility had two or more critical violations.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Repeat Corrected During Inspection Handwashing sink not accessible. Observed hand sink in Meat Dept. that was inaccessible due to cart in front of hand sink.
Action Taken : PIC Removed push-cart.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Corrected During Inspection No towels or drying device at the handwashing sink(s). Observed deli hand sink with no paper towels.
Action: PIC replaced paper towels.
3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Bakery: No thermometer in hot Case to monitor temperature of foods.
Action: PIC corrected violation.
3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Food contact surfaces not easily cleanable. Observed cutting board in Produce Prep area that has deeply ingrained surface on cutting block.
Action: Repair or replace cutting block to provide a clean surface. Correct By: 20-Jan-2020

Facility not maintained clean. -Observed fan filters dirty in Meat Slicing Department -Observed icing around rear freezer door located by the Truck Dock.

Repeat Facility not maintained clean. Observed engrained dirt and grime located at the base of hand wash sinks, located in deli and bakery.

Unnecessary or nonfunctional items and /or litter on premises. Rear storage in Deli: Old dish wash machine that is not in use.
Action: Remove equipment that is no longer in use.

Physical facilities not maintained in good repair. Observed door skin in meat dept. (walk in cooler door) to be frayed and rusted.
Action: Repair door skin to prevent employee injury.
