3717-1-02.4(A)(1) / Person in charge: assignment of responsibility.
Critical There was no person in charge present in the food facility during inspection. Observed Employee did not have PIC training.
To minimize the risk of contamination, the license holder shall be the person responsible for the FSO or RFE, or may be the person in charge, or shall designate a person in charge and shall ensure that a person in charge with applicable knowledge is present during all hours of operation.
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed Chicken tenders in walk in cooler (Lexan) to not have any date marking
Refrigerated, ready-to-eat, TCS food held refrigerated for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day to be consumed, sold, or discarded as required in this rule. Correct By: 27-Apr-2017

There is no test kit available for measuring the concentration of the sanitizer. Observed test papers that have gotten damp and no longer effective to check sanitize measurement.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided. Correct By: 03-May-2017

Critical Equipment food-contact surfaces or utensils are dirty. Observed can opener blade to have dried food debri.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Action: Clean Can open blade between use. Correct By: 03-May-2017
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom. Observed no covered trash receptacle in women's restroom
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. Correct By: 03-May-2017
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. Observed coving missing in front of walk in cooler door and around storage area.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.

The physical facilities are not being maintained in good repair. Observed floor tile in box room to have small chip tin tile.

Observed mops being dried using an unapproved method. Observed wet mop stored in bucket.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Action Wet mops shall be in hanging position after use.
