3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed two employees with the level 1 Person in Charge training but no Ohio Department of Health (ODH) certified person at the facility. Each licensed facility must have at least one person with the ODH level 2 certificate. Submit information to obtain level 2 ODH certificate within one year of approved course at https://odh.ohio.gov/know-our-programs/food-safety-program/resources/manager-certification-app . Obtain by next standard inspection.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed ice bin and a bottle of grenadine at the bar to be uncovered at time of inspection. Ensure all food items are covered to avoid contamination from splash, sneeze, or dust. PIC placed a lid on both the bottle and the ice bin at time of inspection.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw bacon being stored in the walk-in not segregated from ready to eat lunch meats. Ensure raw foods and ready to eat foods are properly segregated to avoid cross contamination. PIC relocated the raw bacon to an acceptable location at time of inspection.

Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed deli slicer with visible residue on the food contact portion of the slicer. PIC had cleaned at time of inspection.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed an opened package of hot dogs (1 lbs), hard boiled eggs (4 eggs), and Virginia ham (1/2 lbs) without a date mark, and PIC verified the date of open was passed seven day expiration date. PIC voluntarily discarded the hot dogs and Virginia ham at time of inspection.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed open package of corned beef (1 lbs) dated 3/2/2023 to be passed the seven day expiration date. PIC had corned beef voluntarily discarded at time of inspection.

Critical Corrected During Inspection Improper storage of poisonous or toxic materials. Observed open oven cleaner being stored on the food preparation surface. Ensure all chemicals are stored away from food storage and food preparation surfaces to avoid cross contamination. PIC relocated oven cleaner to acceptable location at time of inspection.

Corrected During Inspection Working food containers not properly labeled. Observed containers of water being used for the grill to be unlabeled at time of inspection. PIC had containers of water labeled at time of inspection.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed no/improper hair restraints in use at time of inspection. Hair restraints can take the form of hair bands, bandanas, hats, beard/hair nets accompanying complete restraint of hair. Correct by 3/27/2023.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed shelves of the Dasani refrigerator, and bottom shelves of the beer cooler in the main bar and the banquet hall bar areas to be rusted, rendering the surfaces no longer smooth, durable, non-absorbent, and easily cleanable. PIC stated plans to replace the beer coolers, before purchase of beer coolers send specification sheets for specialist review. Repair or resurface by next standard inspection.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available. Observed quaternary ammonium test strips to be expired in 2021. Obtain new test strips by 3/27/2023.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed drip tray placed on the floor under the pop dispenser rack to have visible residue. Clean by 3/27/2023.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed acoustical ceiling tiles in the kitchen that render the surface no longer smooth, durable, and easily cleanable. As the tiles become soiled or damaged, replace with smooth and easily cleanable tiles.

Facility not maintained clean. Observed ceiling of walk-in cooler to have visible black residue accumulation. Clean by 3/27/2023.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Light intensity less than fifty foot candles in required areas. Observed 35 CFP (candle foot power) light intensity in the kitchen over the warewashing unit and the food preparation area. Ensure areas receive at least 50 CFP in these areas by next standard inspection.
