Equipment components are not intact, tight or properly adjusted. Observed Kolpak door that appeared to be warped.

Corrected During Inspection Equipment and/or components not maintained. Observed smell reach in cooler(right side) reading 50 degrees. Content were discarded and unit was taken out of service.

Critical Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed improper back flow for high pressure nozzle.
