PIC unable to locate signed copies of employee illness reporting agreements at the time of inspection. Provided RPH's updated employee illness reporting agreement form at the time of inspection.

Corrected During Inspection No written procedures for responding to vomiting or diarrheal events. Provided ODH's guidance for clean-up of vomit/fecal accidents.

Critical Repeat Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Observed food debris in the front hand washing sink. PIC removed debris at the time of inspection.

Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed sliced turkey in the make table cooler by the drive thru at 48*F. PIC voluntarily discarded turkey at the time of inspection.

Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed a container of ham in the front cooler without a date mark. PIC was unsure when product had been opened. PIC voluntarily discarded ham at the time of inspection.

Repeat Improper storage of food items. Observed boxes of food stored on the floor in the walk-in freezer. Correct by: 9/8/20.

Corrected During Inspection In-use utensils improperly stored. Observed the handle of the ice scoop in contact with ice in the ice machine. PIC removed ice scoop at the time of inspection.

Clean equipment and utensils not stored in a self-draining position and covered or inverted. Observed bowls above the three compartment sink and metal pans on the dishwasher not covered or inverted to protect from contamination. Correct by: 9/8/20.

Corrected During Inspection Single-service and single-use articles not protected from contamination. Observed single-use trays not covered or inverted to protect from contamination. PIC placed trays back into plastic bag during inspection.

Nonfood-contact surfaces not easily cleanable. Observed the sealing/coating on the shelves missing/peeling in several locations in the walk-in cooler. Correct by next inspection.

Equipment not approved by a recognized testing agency. Observed a non-commercial Magic Chef microwave. When equipment no longer functions properly, replace with commercial grade.

3717-1-04.1(DD) / Warewashing machines - automatic dispensing of detergents and sanitizers.
Critical Warewashing machine does not automatically dispense detergent sanitizer and/or does not have a visual means to ensure detergent and sanitizer is automatically dispensed. Observed a container of liquid chlorine sanitizer mounted on the wall above the dishwasher. No sanitizer observed in the lines at the time of inspection. No sanitizer detected in dishwasher when operated at the time of inspection. Discussed using the three compartment sink with the quat sanitizer until dishwasher is repaired to dispense chlorine sanitizer at 50 ppm or above. Correct by: 9/8/20.

No sanitizer test kit available. No bleach test strips available to determine the concentration of the sanitizer used in the dish washer. Correct by: 9/8/20.

Non-food contact surfaces of equipment are unclean. Observed residue on the inside of several Sterilite plastic containers holding clean utensils. Correct by: 9/8/20.

Discussed inspection with managers Shannon and Seth. Facility has obtained a new commercial Standex make table cooler by the drive thru: model #SMPWT60-24-30. Re-inspection to occur on September 8, 2020. Emailed inspection report.
