Facility does not have an employee with manager certification in food protection. Destiny Sigler stated she has been Certified in Food Protection, however; she was unable to provide documentation. Correct by next inspection.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Repeat No written procedures for responding to vomiting or diarrheal events. PIC was unable to provide procedures or kit. Emailing procedures. Please put together a kit for clean-up. Correct by next inspection.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. Not thermometer in the large make table cooler. PIC called someone to bring one in today.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface(s) not cleaned at the required frequency. Exhaust vent filters over the the pizza oven have visible build-up on them. Correct by next inspection.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Mop sink drain was plugged up. PIC stated a repairman had already been called. Correct by next inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Visible dust build-up on the wall beside the pizza oven. Correct by next inspection.
