PIC unable to locate signed copies of employee health policies at the time of inspection. Provided RPH's employee illness reporting agreement form and PIC signed and posted at the time of inspection.

Critical Repeat Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw bacon on a shelf above canned drinks inside of the walk-in cooler. PIC moved bacon to the bottom shelf inside the cooler at the time of inspection.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed cartons of raw shell eggs stored on the shelf above ready to eat foods inside the reach-in sandwich cooler. PIC moved eggs to the bottom shelf inside of the cooler at the time of inspection.

Critical Corrected During Inspection Improper storage of poisonous or toxic materials. Observed gallons of windshield washer fluid stored on a shelf above canned drinks in the back storage area. PIC moved windshield washer fluid to the bottom shelf below drinks at the time of inspection.

A food packaged in the facility not labeled. Observed a bag of mini tacos in the freezer without a list of ingredients, net weight, or address of packaging facility. Correct by: 3/28/22.

3717-1-03.5(C)(2)(e) / Food labels - label information
Critical Repeat Corrected During Inspection Missing and/or incomplete allergen declaration(s). Observed sleeves of Ritz and Saltines crackers for sale without a list of ingredients, common name, net weight, or address of packaging facility on the package. PIC removed crackers from sale at the time of inspection.

Repeat A food packaged in the facility not labeled. Observed packages of Fruit by the Foot snacks labeled not for individual retail sale and no list of ingredients on the package. Correct by: 3/28/22.

Physical facilities not maintained in good repair. Observed a missing floor tile in the bagged chips and snacks aisle. Correct by next inspection.

Repeat Facility not maintained clean. Observed residue on the blue door inside of the walk-in beer cooler. Correct by next inspection.

901:3-4-15(B) / Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
Critical Corrected During Inspection Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired. Observed two 12.6 oz. cans of Nutramigen formula with a use by date of Feb. 1, 2022. PIC voluntarily opened cans and discarded formula into the trash can at the time of inspection.
