3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed improper storage of food items. Observed three baguettes sitting on a prep surface near the walkway (not being actively worked with) to have no protective covering against sneeze, splash, landing insects, and dust. PIC added parchment paper on top to protect.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection TCS foods were not being held at the proper temperature. Observed raw sliced tomatoes in the sandwhich prep cooler (which were removed from cold holding before slicing) to be above maximum safe cold holding temperature. Product pans (2 total) measured 45.1*F and 47.2*F at time of inspection. PIC voluntarily discarded tomatoes.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
3717-1-04.4(B) / Cutting surfaces.
Repeat The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. Observed the cutting board on the panini prep table to be badly scored, stained, and chipped.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced. Correct By: 30-Jan-2018
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. Observed accumulation of dried food residue on the counter-mounted can opener blade. PIC removed to mechanical warewasher for cleaning at time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(A)(2) / Cleanliness of food-contact surfaces of cooking equipment and pans.
Repeat Cooking equipment or pan surfaces are dirty. Observed significant creosote build-up on baking racks and pans. PIC states that they are handled in the following manner: once per year in the spring, a local third party company is contracted to deep clean/remove buildup from all named equipment in the facility. The state of equipment at time of inspection indicates this process is due. If build-up has reached a level indicating greater frequency of this process is required, ensure the current once-yearly deep clean schedule is increased. Correct by next standard inspection, on or following March 1st, 2018.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. Observed the outter surfaces of microwave ovens to be in need of greater cleaning frequency. Observed hard water scale build-up in the food preparation sink sprayer nozzle. Observed syrup splash build-up and mildew growth on the underside of the customer self-service soda fountain.
Nonfood-contact surfaces of equipment shall be kept clean. Correct By: 30-Jan-2018
3717-1-04.7(B) / Frequency of laundering - specifications.
Linens, cloth gloves, and wiping cloths were not properly laundered. Observed a pair of oven mitts which were excessively soiled with visible food residue and particles.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Cleaned equipment and utensis are not properly stored. Observed the freezable drop-in quarter pans in the walk-in freezer to not be covered or inverted to protect the food-contact surfaces.

3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
The plumbing system was not properly maintained. Observed the service sink (mop sink) atmospheric vacuum breaker to be broken. Cover is missing, plastic inner parts are exposed.
