Action Provide documentation of a certified manager.
3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Can opener blade dull or creating metal fragments. Observed dirty can-opener blade with coating burnished.
Action: Clean can opener blade between use, replace blade when needed.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed open hole in floor located in front of three compartment sink.
Action: Keep floor covers in place.
3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed door skin inside lower walk-in cooler door with ripped skin.
Action: Repair door to provide smooth cleanable surface.
