Critical Corrected During Inspection PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating. -PIC and employees could not correctly answer questions regarding proper cooling time/temperature parameters and methods. Discussed proper cooling procedures and provided a cooling handout via email with report.

Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. -Observed a container of applesauce on the counter with no cover and not being actively prepared or used. PIC placed a cover on the applesauce.

3717-1-04.5(B)(5) / Equipment food-contact surfaces and utensils - cleaning frequency.
Utensils and equipment contacting non-TCS foods not cleaned at required frequency. -Observed the apple cider slushy dispenser nozzle to contain black residue.

3717-1-03.4(D) / Cooling - temperature and time control.
Critical Repeat Corrected During Inspection Improper cooling of TCS food. -Observed 3 applesauce containers that had been prepared the day before to be at 45*F, 83*F and 86*F. PIC states the product is cooked, placed on the counter to cool for a little, poured into the large gallon containers and then placed in the cooler with a lid. PIC voluntarily discarded 5 gallons of applesauce. Discussed proper cooling time and temperature parameters and cooling methods.

3717-1-03.4(E) / Cooling methods - temperature and time control.
Repeat Improper method for cooling TCS foods. -Observed multiple gallon containers of applesauce in the cooler to not be properly cooled. Observed the product in large, deep pans with a lid. Discussed proper cooling methods with PIC. Provided a cooling handout via email with report.

Repeat Packaged food labels are not in compliance with standards of identity. -Observed eggs from facility farm to be labeled in re-used cartoons with the previous information still visible.

Critical Presence of live insects. -Observed many flies in the bakery kitchen, near display cases and in store front near cider dispensers. Discussed with PIC, who states the facility will take additional measures to control flies and contact a pest control operator.

Food employee(s) hair not effectively restrained. -Observed both employees preparing bakery products to not effectively restrain hair by wearing a hat, hair net, etc.

Equipment not approved by a recognized testing agency. -Observed noncommercial equipment being used: Kenmore freezer/refrigerator unit, Oster toaster, Haier mini fridge, and multiple crock-pots and roaster. If/when equipment no longer functions, replace with commercial grade.

Fixed equipment not properly sealed or spaced for cleaning. -Observed the handwashing sink to not be properly sealed to the wall creating a space difficult to clean.

Critical Corrected During Inspection Food contact surfaces not easily cleanable. -Observed a bowl of apple slices to be covered with a cloth. PIC removed cloth and place was paper over apples.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Non-food contact surface(s) not cleaned at the required frequency. -Observed apple walk-in cooler to contain a build-up of dust/mildew on ceiling and condenser unit.

Outdoor refuse storage surface not properly constructed and sloped to drain. -Observed the outside dumpster to be set on gravel and not on a smooth, durable surface.
