3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed clean tongs hanging on wall in contact with wall, rendering the food contact surface clean. PIC had tongs sent to warewashing area at time of inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed 0ppm chlorine sanitizer in the 3 compartment sink. Ensure chlorine sanitizer maintains 50-100ppm during usage. PIC corrected at time of inspection.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Corrected During Inspection Presence of live rodents. Observed droppings in storage closet. PIC had cleaned at time of inspection.

3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Presence of dead pests or insects. Observed dead insects in the light shield throughout the facility. Remove by next standard inspection.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Corrected During Inspection [51] Improper storage of cleaned equipment, utensils, and laundered linens. Observed cleaned tong hung on wall in such a manner that the food contact surface was touching wall. Ensure clean equipment and utensils are stored in such a manner that no contamination may occur. PIC corrected at time of inspection.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed shredded chicken scoop to be melted and cracked, rendering the food contact portion not smooth, durable, not absorbent, or easily cleanable. PIC voluntarily discarded scoop at time of inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed floors in the freezer to have visible residue and ice. PIC removed ice at time of inspection, clean floors by reinspection 3/26/2024.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed visible residue on the tracks of the lobby pop cooler. PIC cleaned at time of inspection.

3717-1-05.1(E) / Backflow prevention device - design standard.
Critical Improper backflow prevention device installed on a water supply system. Observed no backflow on the spray nozzle near the custard machine. Ensure either a ASSE 1011 hose bib vacuum breaker is installed properly. PIC stated one has been ordered. Correct by reinspection 3/26/2024.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed various portions of the floor throughout the facility with chipped or cracked tiles, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC stated plans for replacement later this year. Correct by next standard inspection.

Conducted and reviewed inspection with PIC.
Facility's risk level 3 activities include: cook/hold of TCS products. No reheating in bulk for hot holding occurring at this point in time.
