Violations: A summary of the violations found during the inspection are listed below.
3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. -Observed a pitcher of chicken broth and beef broth stored on the counter in an ice bath at 47*F and 52*F. The ice water was below the level of the broth. PIC voluntarily discarded products. -Observe multiple single portion containers of chicken salad stored in the customer self serve cooler to be at 44-46*F. PIC stated the product had been prepared and portioned this morning, stating the chicken is pre-chilled (41*F), however they add room temperature products such as mayonnaise and pineapple. The product was not cooled to 41*F or below after adding the room temperature ingredients. PIC moved all products to the walk-in cooler to be properly cooled for cold holding.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. -Observed gallons of open milk that were not properly date marked. PIC voluntarily discarded 1/2 gallon of 2% milk and 1/4 gallon of 1% milk. -Observed 4 pans of sloppy joe in the walk in cooler date marked to be used by 1/18/19. PIC stated the product was prepared and frozen on 10/21/18 and were pulled from the freezer to be thawed in the walk-in cooler on 10/28/18. PIC stated the 1/18/19 use by date is that date the product is to be pulled from the freezer. PIC properly labeled the product for 6 days after thawing (with day 1 being date pulled from the freezer).

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection In-use utensils improperly stored. -Observed a scoop stored in a bin of oats with the handle in direct contact with the food product. PIC removed scoop and placed in dish area.

3717-1-04.8(E)(1) / Equipment, utensils, linens - storage.
[51] Improper storage of cleaned equipment, utensils, and laundered linens. -(CORRECTED DURING INSPECTION) Observed a pair of scissors stored in a drawer with clean utensils used for food. PIC moved scissors into separate drawer. -Observed cleaned serving utensils (in clean dish area and serving areas) to be stored in a disorganized manner, with handles not facing the same direction. PIC stated he will work on reorganizing clean utensil bins/drawers and storage area.

Equipment and/or utensils improper construction. -Observed multiple spatulas to have melted/warped handles.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Corrected During Inspection Nonfood-contact surfaces constructed of unapproved materials. -Observed bread ties around the handles of multiple small frying pans. PIC removed bread ties from pans.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components not maintained. -Observed the True single door freezer in the serving line area to be missing the bottom covering/kickboard. -Observed the end weight component of the meat slicer to have several broken/missing nipples. -Observe a lower cupboard door in the coffee area underneath the coffee urns to be in disrepair on the inside surface causing the surface to no longer be smooth and easily cleanable.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. -Observed a food scale stored on a lower shelf of a prep table to contain what appeared to be flour on the outside surface. -Observe multiple small frying pans in the clean dish area to contain a build-up of black carbon residue around the outside of the handle. -Observed insulation lines of the steam kettles to contain a build-up of old food residue. -Observed cooking/baking equipment throughout the facility in need of greater cleaning frequency. Multiple cooking/baking units contained an accumulation of black carbon residue and old food debris. Special attention to ovens (in serving line and cafeteria) and stove burners. -Observed multiple reach-in freezer and cooler units to contain food debris spillage on the bottom shelf.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. -Observed the metal kickboard behind the steam kettles to be crooked and no longer attached to the wall. PIC stated they currently have a work order in place to correct the issue. Correct prior to next inspection.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Ventilation system not maintained. -Observed air ventilation system above the serving line to contain an accumulation of dust. PIC states they are aware of the issue and are working to find a time after hours (when food is not being prepped or served) to clean the vents and surrounding ceiling area.
