Observed food packages that are not properly labeled. OBSERVED: foods in reach in cooler (sandwiches, deli salads) Bagels, Cookies, deserts that were not properly labeled. All items for self service shall be labeled per the code & include, all ingredients, sub ingredients, net weight & manufacturer to prevent a food allergy to individuals which could result in death
To properly inform consumers, packaged food shall be labeled as specified in law. Correct By: 10-Feb-2016
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. OBSERVED:Raw hamburger on a cart above raw onions, fresh chopped celery/tomatoes in the meat cooler. To prevent a foodborne illness , raw meats shall be stored below or separate from raw ready to eat foods.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. CORRECTED: Manager moved hamburger to an appropriate storage spot

NOTE: As of March 1, 2017 facility will need a Management staff with level 2 training per the New updated Ohio Food Food with an Ohio certificate and a written employee health policy detailing when employees are sick & when to stay home & report their illness & have an incidence report for vomiting/diarrheal accidents. They facility will also need a thermal indicator ran through the dishwasher daily to show the hot rinse is working correctly.
