Critical Corrected During Inspection PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage. Observed multiple TCS products not properly cold or hot held. PIC corrected temperature violations. Reviewed proper cold and hot holding temperatures.

3717-1-02.4(C)(16) / PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Critical Written procedures and plans are not maintained and implemented. Facility has written procedures for sliced American cheese variance, however procedures were not being followed or monitored properly. PIC stated facility is using time-in-lieu of temperature for multiple TCS products, however no procedures were on file and products were not time marked. Provided time-in-lieu of temperature procedure for PIC to complete and keep on file. -Correct by 4/1/22

Critical Corrected During Inspection PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating. PIC was unable to correctly answer questions regarding cold holding, hot holding, and cooling and reheating times and temperatures. Observed multiple products incorrectly held and reheated. Reviewed proper times and temperatures. PIC corrected all temperature violations.

Critical Corrected During Inspection PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. Observed sanitizer solution concentration at 100 ppm when filled. PIC stated solution was mixed with hot water of 200*F. Reviewed proper sanitizer concentration and temperature parameters. PIC corrected sanitizer concentration to bring to 400 ppm.

Corrected During Inspection Employee eating, drinking, or using tobacco in non-designated area. Observed personal beverages stored over food products served at facility in walk-in cooler. PIC moved personal beverages to lowest shelf separated from other products.

Critical Corrected During Inspection Food employee(s) did not wash hands when required. Observed employee adjust hair and then start to put on single-use gloves without first washing hands. Employee removed gloves and washed hands.

Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed sanitizer solution near registers at 100 ppm. PIC replaced solution to bring to 400 ppm. Discussed proper concentration and temperature of sanitizer solution.

Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed tomato slicer with old food residue. PIC placed slicer is dish area to be properly washed, rinsed and sanitized. Reviewed cleaning frequency of slicer.

Critical Corrected During Inspection Improper reheating of food for hot holding. Observed chili being reheated at 142-158*F. PIC states reheating was started at 8am, product was temped around 11am. Reviewed proper reheating parameters. Ensure product is rapidly reheated to 165*F within 2 hours. PIC voluntarily discarded product, see voluntary destruction form.

Critical Repeat Corrected During Inspection TCS foods not being hot held at the proper temperature. Observed chili burgers hot held at 119*F and 121*F in two drawer units. PIC voluntarily discarded product, see voluntary destruction form. Employee states dial on unit is not working properly. PIC instructed employee to hot hold new product in a functioning unit.
