Critical Corrected During Inspection Person in charge did not ensure their employees are properly trained in food safety. -Facility does not have a person level 2 certified in food safety and observed multiple critical violations during time of inspection. Employee corrected all critical violations at time of inspection.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have a person with level two certification in food protection. -PIC states she has taken the online course and is waiting to take the proctored exam. Correct prior to next standard inspection.

3717-1-02.4(C)(14) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. -Provided RPH's employee reporting illness agreement. PIC stating they will make copies and have each employee sign.

Critical Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. -Observed dirty wiping cloths stored inside the handwashing sink. PIC removed wiping clothes. -Observed employee draining fruit salad into handwashing sink. Employee completed draining fruit salad in prep sink.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. -Observed fruit salad stored on customer self service line to be at 56*F and the bulk container of fruit salad in refrigerator to be at 45*F. PIC voluntarily discarded 5 cups of fruit salad. -Observed waffle batter in refrigerator to be at 61*F. PIC states waffle batter was just prepared and placed in freezer to rapidly cool. -Observed food products stored in upstairs refrigerator to be at 42*-43*F. PIC removed all TCS products from upstairs refrigerator and placed in refrigerator in kitchen.

Critical Corrected During Inspection Improper storage of poisonous or toxic materials. -Observed coffee filters stored in a bin with chemicals. PIC removed coffee filters.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Inadequate equipment available for cooling, heating or holding food. -Observed refrigerator/freezer unit upstairs to be at an ambient temperature of 46*F and food products to be 42*-43*F. PIC turned unit temperature down. Correct prior to using for foods served at facility.

Critical Repeat Corrected During Inspection Insufficient air gap between the flood rim and the water supply inlet. -Observed a black pipe being used as a funnel to catch splash from the prep sink drain creating no effective air gap. PIC removed funnel.

3717-1-06(A)(1) / Indoor areas - surface characteristics
Corrected During Inspection Floors, walls, and/or ceilings not constructed of approved materials. -Observed duct tape placed over a drain. Maintenance personnel removed duct tape from floor drain.

Facility is using a noncommercial refrigerator/freezer unit located upstairs for storing overflow products. During initial inspection, ambient air of the unit was at 46*F. Employee removed all TCS foods from unit and turned the temperature down. After about 2 hours the ambient air was at 39*F. Sanitarian will follow up to verify if unit is approved for continued use. Ensure a thermometer is placed inside unit and it is kept at 41*F or below if storing TCS food products for facility inside unit.
