3717-1-03.2(Y) / Miscellaneous sources of contamination.
Corrected During Inspection Miscellaneous sources of contamination observed. Observed containers/packages of food stacked on top of frozen food products in reach in drawers.
Food shall be protected from contamination that is not specified under 3717-1-03.2 (A) to (X). To avoid contamination of food products disconinue stacking on top of unprotected foods. PIC voluntarily threw away 6 cinna poppers that had been under a bag of veggie burgers.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. Observed far right prep cooler to contain water accumulation on inside.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair inside of cooler.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. Observed lettuce shredder to contain old lettuce, can opener, electric mixer to be put away dirty, and meat slicer to have old meat particles.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean all food contact surfaces of equipment.Equipment was cleaned during inspection.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. Observed inside top of ice bin in bar area to contain mold on lip.
Nonfood-contact surfaces of equipment shall be kept clean. Clean ice bin lip.
3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Corrected During Inspection Food equipment surfaces are not cleaned at the required frequency. Bowls used to toss chicken wings were not being washed, rinsed and sanitized at the required frequency; PIC stating after lunch around 4pm, facility opens at 11am.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule. Educated over proper cleaning frequency of chicken tossing bowls and other equipment/utensils in contact with TCS foods (tongs, knives, breading bowl, etc.) need to be washed, rinsed and sanitized every 4 hours at maximum to prevent bacterial growth. Bowls were pulled and switched with new.
3717-1-04.5(C) / Cooking and baking equipment - cleaning frequency
Cooking and/or baking equipment is not cleaned at the required frequency. Observed pizza and quesadilla pans to have black carbon residue.
Cooking and baking equipment and/or cavities and door seals of microwaves shall be cleaned at least every twenty-four hours. Clean pans.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. Observed warewashing racks to contain soiled residue accumulation.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean warewashing racks.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. Observed fryer insides to contain grease residue accumulation.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. Observed covering to be missing in walk in and surrounding.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. Replace coving in facility.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. -Observed walk-in freezer to have stilgmites forming from lines. - Observed door frame to walk in freezer to be detached creating ice accumulation.
The physical facilities shall be maintained in good repair. Properly repair walk in freezer.
