Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. -Observed the can opener to contain food residue and metal shavings. PIC placed in dish area to be properly cleaned and sanitized.

3717-1-03.4(F)(1)(a) / Time/temperature controlled for safety food - hot holding.
Critical Corrected During Inspection TCS foods not being hot held at the proper temperature. -Observed a bowl of broth setting at room temperature to be at 115*F. Employee stated the broth was made around 10:30am and was set on the counter to be served at lunch at 11:30am. Employee voluntarily discarded 1 bowl of broth.

3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. -Did not observe a thermometer in the walk-in cooler. PIC properly placed a thermometer in the walk-in cooler.

3717-1-04.2(H)(2) / Temperature measuring devices - mechanical warewashing.
Repeat Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. Correct by 12-17-19

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. -Observed multiple clean utensils and containers/pans to contain tape, labels and/or label residue on the outside portion. Discussed removing label prior to running dishes through the dish machine. Correct by 12-17-19

No covered receptacle in women's rest-room. -Observed trash receptacle in employee rest-room to be missing a lid. Correct by 12-17-19

Toilet room did not have a self-closing door. Correct by 12-17-19

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. -Observed the wall behind the dish machine and dish counter to contain food splash. Correct by 12-17-19

-Ensure coffee filters remain covered. Initially observed coffee filters to not be covered, employee properly covered coffee filters without being prompted.

Facility is a risk level IV due to cooling and reheating in bulk and serving a highly susceptible population.
