Facility does not have an employee with manager certification in food protection.

3717-1-06.1(K) / Insect control devices - design and installation.
Improper use or placement of insect control devices. Observed glue fly strips improperly used in kitchen area.
Action Remove fly glue fly strips. Note: Blue light/glue strips are approved to be used in kitchen.
3717-1-06.4(K) / Controlling pests.
Critical Presence of live insects, rodents, and other pests. Observed flies throughout kitchen.
Action: Screen doors shall remain closed during operation.
3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) not wearing a hair restraint. Observed not maintained hair.
Required ball caps, hair nets, fully maintained hair.
3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed heavily scored cutting boards.
Action: Provide smooth easy to clean surface.
3717-1-04.4(D) / Warewashing equipment - cleaning frequency.
Warewashing equipment and/or components not cleaned at required frequency. Inside Wash machine heavy lime residue on walls.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Observed wash basket with heavy lime residue on basket.
Action: Replace baskets as necessary, to provide a clean surface.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Observations: -Heavy dust and residue on light fixtures, mainly above fryers. -Fan shield with dust/dirt on shield. -Saw dust pile in basement near Soda/Pop syrup jackets. Clutter through out basement. -Hard to clean area above walk in cooler and refrigeration units have heavy dirt/dust build up. - Provide screens/covers on equipment where motors are exposed.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observations: -Ceiling tile missing through out facility.

3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat No protective shielding on lights. Observation: -Light shields missing through out kitchen.

3701-21-25(I) / Person in charge certification in food protection
Repeat FSO did not have a person in charge per shift with the person in charge certification in food protection.
Employee need document person in charge training certificate's. Required per shift.

Conducted inspection and reviewed inspection findings with PIC. Risk Level IV activities include: Offers raw menu items, reheats bulk quantity of leftover TCS foods, Cook Cool/ Reheat/Hot Hold. Provided educational material.
