3717-1-03.2(D) / Food storage containers - identified with common name of food.
Corrected During Inspection Food storage containers are not properly labeled. Observed several containers bearing no common name label of their contents.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04(A) / Multiuse utensils and food contact surfaces - material characteristics.
Equipment and/or utensils contain improper materials. Observed large grey trash receptacle holding bulk quantity of flour. If using, must place food in a food-grade plastic bag as a liner, so that food is not in direct contact with non food-grade container.
To prevent contamination, materials that are used in the construction of utensils and food contact surfaces may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: Safe; durable, corrosion-resistant, and nonabsorbent; Sufficient in weight and thickness to withstand repeated warewashing; finished to have a smooth, easily cleanable surface; and resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. 3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed a white non-commercial grade Haier upright freezer and white Lazarus chest freezer.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. When equipment is no longer functional, replace only with commercial-grade equipment.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. Observed dried food build-up on the can opener blade and can opener holder.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of the white Lazarus freezer have a buildup of ice on the interior; top, bottom, and sides.

Observed crumbs/food debris buildup on and beneath the pizza oven racks.

Discussed inspection with Judy Gleason, PIC.

PIC has signed copies of employee illness reporting agreement maintained at facility. Continue to have new employees sign one as well.
Provided written cleanup procedures for a vomit or diarrhea incident. Provided norovirus fact sheet and clean up kit assemble guide.
Assemble a clean up kit and keep in an accessible location in the facility, along with written clean up procedure document provided.

Discussed increasing air gap beneath prep sink per 3717-1-5.1(D) Backflow prevention - air gap.
"An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (twenty-five millimeters)."

Air gap present beneath prep sink, however sanitarian observed discharge pipe to be touching the catch cup directly.
