3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Observed employee eating, drinking, or using tobacco in non-designated area. Observed two (2) bottled personal beverages on the prep table in the kitchen area. Personal drinks must include both a lid and a straw if allowed by management.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result. Correct By: 26-Jan-2018

Non-food contact surfaces of equipment are dirty. Observed the glass doors and track of the Coke cooler holding chicken to be in need of cleaning.

The Ohio Uniform Food Safety Code:
3717-1-5.1(D) Backflow prevention - air gap.
"An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (twenty-five millimeters)."

Air break is present beneath prep sink, however sanitarian observed discharge pipe to be touching the catch cup directly.
This issue was first noted during a Standard Inspection on 11/06/2017.

*Drain pipe must be trimmed higher to be in compliance. Correct by next Standard Inspection.*

PIC has signed copies of Employee Illness Reporting Agreement in a binder.
-Continue to have any new employees sign one and keep these on the premises at all times.

PIC has RPH Vomit Response Procedure present.
-Post on the wall in facility or keep in an easily accessible location.
-Ensure all supplies listed under step #2 of the above document are contained in commercially-prepared clean up kits (which PIC states facility intends to obtain).
