Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed a container of ice for infused water in the freezer that was uncovered. PIC covered ice with plastic wrap at the time of inspection.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. Observed a bucket of sanitizer solution at 100 ppm. PIC refilled bucket with solution at 200 ppm at the time of inspection.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed butter packets in a container above the make table cooler at room temperature. PIC voluntarily discarded butter packets at the time of inspection.

Corrected During Inspection Label on a food packaged in-house did not contain all required information. Observed packages of string cheese in the grab n go cooler without a list of ingredients. PIC removed string cheese from cooler at the time of inspection.

Corrected During Inspection [52] Improper storage of single-service and single-use articles. Observed a box of single use bowls on the floor in the storage room. PIC moved box off the floor at the time of inspection.

3717-1-04.4(I) / Manual warewashing equipment - wash solution temperature.
Critical Corrected During Inspection Manual wash solution not being maintained at 110˚ F or above. Wash solution in the three compartment sink was at 101*F. PIC brought solution up to 110*F at the time of inspection.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed dust on the bottom vents of the grab n go cooler. PIC cleaned vents at the time of inspection.

FSO license not displayed. Correct by next inspection.

Corrected During Inspection Food choking standards not readily accessible in FSO. Provided a first aid for food choking handout at the time of inspection.

Discussed inspection with kitchen manager Maranda Powell. Discussed date marking of opened milk if kept for more than 24 hours. Discussed cooling and reheating in bulk of one food item once per week under a Risk Level 3 license. Facility is using an ice bath to cool soup once per week. Left a cooling foods safely handout and cooling record log. Left an extra copy of RPH's employee illness reporting agreement form. Discussed having new food employees sign upon hire and keep copies on file in the facility for review. Emailed inspection report.
