Critical Corrected During Inspection PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating. During discussion, PIC could not state proper hot holding temperatures. Ensure hot holding occurs above 135F at all times. Education provided at time of inspection.

3717-1-02.4(C)(2) / PIC: duties - ensure unnecessary persons are not allowed in food prep or storage areas.
Critical Corrected During Inspection Unnecessary persons in the operation. Observed a bar patron walking through kitchen and bar area multiple times during active prep and service. PIC verified they are not on staff, or volunteering at time of observation, only a patron. Ensure no unauthorized persons or employees/volunteers not on shift have access to kitchens or bars. Person exited kitchen and education provided at time of inspection.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection. Observed no Ohio Department of Health (ODH) certified person at the facility. Each licensed facility must have at least one person with the ODH level 2 certificate. Submit information to obtain level 2 ODH certificate within one year of approved course at https://odh.ohio.gov/know-our-programs/food-safety-program/resources/manager-certification-app . Obtain by next standard inspection.

Critical Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. Observed handwashing sink in the back kitchen area to be used for storage. Ensure nothing other than handwashing is occurring in the handwashing sink. PIC removed items at time of inspection.

Critical Repeat Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw bacon stored above ready to eat foods. Ensure raw foods are stored below ready to eat foods at all times. PIC had relocated to appropriate location at time of inspection.

Critical Repeat Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed pitchers of mixed drinks to be in the dasani cooler without coverage, rendering the drink exposed to sneeze, splash, and dust. PIC voluntarily discarded two pitchers of mixed drink at time of inspection.

Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed can opener blade to have visible residue. PIC stated that can opener is no longer in use, and relocated out of use can opener to storage closet at time of inspection.

Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed french fry slicer to have visible residue. PIC had parts sent to warewashing at time of inspection.

Utensils and equipment contacting non-TCS foods not cleaned at required frequency. Observed soda dispensers at both bars to have visible residue. PIC had the one at the front bar cleaned, the back bar was left untouched. Clean by 2/21/2024.

Critical Repeat Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed tomatoes and various breakfast items to be stored in the walk-in without a datemark. Ensure all items receive a datemark 24 hours after opening and storage. PIC had all datemarked at time of inspection.

Critical Repeat Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Observed cole slaw (2/3) and corn beef (2/2) passed datemark. Ensure all foods are discarded after 7 total days. PIC voluntarily discarded both at time of inspection.

Critical Corrected During Inspection Improper storage of poisonous or toxic materials. Observed dish soap stored with cleaned bowls above the warewashing unit in the small kitchen. Ensure all chemicals are stored separately from cleaned equipment and foods. PIC relocated soap to appropriate location at time of inspection.

3717-1-07.1(J) / Restricted use pesticides - criteria.
Corrected During Inspection Unapproved pesticide being used. Observed wondercide pest small fly trap being used and applied by facility. This is not commercial use or approved to be used in a kitchen. Ensure all pest control products are applied by a licensed pest control operator and are approved for use within a commercial kitchen. PIC voluntarily discarded at time of inspection.

Corrected During Inspection Food not protected from contamination. Observed pizza served to members only for free at an event being stored in the walk-in cooler above products for sale. PIC stated that this pizza is not be sold to patrons.Ensure all personal items are stored below or completely separated from foods and products for sale. PIC voluntarily discarded pizza at time of inspection.

Nonfood-contact surfaces constructed of unapproved materials. Observed cardboard under the chemical shelving in the mop closet, rendering the surface no longer smooth, durable, non-absorbent, or easily cleanable. Remove by 2/21/2024.

Corrected During Inspection Non-food contact surfaces of equipment are unclean. Observed fryer baskets to have visible residue. Ensure baskets are cleaned at a 24 hour cleaning frequency if held. PIC had sent to warewashing area at time of inspection.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed acoustical ceiling tiles in the small front kitchen, also observed water damage on some of those tiles. When tiles become damaged, replace with tiles that are smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

Repeat Facility not maintained clean. Observed ceiling and floor in the walk-in cooler under the crates to have visible residue. Clean by 2/21/2024.

Conducted and reviewed with PIC.
Facility's risk level 4 activties include: cook/cool/reheat of TCS foods in bulk more than once per week.
Observed ROP machine within the facility, prior to use of ROP machine facility is to submit approved HACCP plan to RPH.
Emailed inspection.
