Critical Corrected During Inspection PIC unable to demonstrate knowledge of relationship of food safety and safe and adequate equipment. Through discussion, PIC could not stated how to calibrate food thermometers. Education provided during inspection.

Facility does not have an employee with manager certification in food protection. Observed no Ohio Department of Health (ODH) certified person at the facility. Each licensed facility must have at least one person with the ODH level 2 certificate. Submit information to obtain level 2 ODH certificate within one year of approved course at https://odh.ohio.gov/know-our-programs/food-safety-program/resources/manager-certification-app . Obtain by next standard inspection.

Cooking and/or baking equipment not cleaned when required. Observed dried residue on the roof of the microwave. Clean by reinspection 10/25/2023.

Critical Corrected During Inspection TCS foods not being hot held at the proper temperature. Observed meatballs being held on the from line at 120F for an undetermined amount of time. Ensure all hot held items are holding at 135F at all times. PIC voluntarily discarded 2.5 lbs of meatballs and sauce at time of inspection.

Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed the following items out of proper cold holding temperature parameters: 5 lbs sliced tomatoes (45F-47F), 2 lbs capicola (50F), and 1 lb shredded lettuce (52F). Ensure cold holding temperature fall 41F or below at all times. PIC voluntarily discarded all at time of inspection.

Critical Presence of live insects. Observed small black flying insects in the following locations: front line wall near BOH entrance door, under prep and 3 compartment sink, and under the soda fountain cabinet. PIC stated the facility is serviced by pest control quarterly. Remove all evidence of pest and contact licensed pest control operator and retain records by reinspection 10/25/2023.

Non-durable equipment observed. Observed ambient temperature of serving line cooler to be at 52.8F and unable to hold product at the intended temperature. Discontinue use of unit for TCS products until it can reliably hold product at correct temperatures. PIC stated plans are in place to get the unit serviced. Correct by 10/25/2023.

Repeat Non-durable equipment observed. Observed dishwashing machine not in use due the unit tripping the electric breaker every time it is in use. PIC stated there are plans in place to fix the unit. Correct by next standard inspection.
