Observed various areas of the floor deteriorating so the surfaces are rendering no longer smooth, durable, non-absorbent, and easily cleanable. PIC stated a plan is in place and contractors have been contacted since last inspection, yet are booked a year from now. Correct by next standard inspection.

3717-1-06.4(K)/Controlling pests. Presence of rodents. Observed rodent droppings in the back hot water tank storage closet. Completely remove all evidence of pests, contact a licensed pest control provider, and thoroughly clean the area by reinspection 2/23/2023
