3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) did not have their hair effectively restrained. At time of inspection, female employee was observed assembling french fries and sandwiches without wearing proper hair restraints
To prevent contamination, food employees shall effectively restrain or cover hair to keep their hair from contacting exposed food, clean equipment, utensils or linens or unwrapped single-service or single-use articles.
3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Corrected During Inspection Observed improper use and/or maintenance of wiping cloths.
(2) Cloths that are in use for wiping counters and other equipment surfaces shall be: (a) held between uses in a chemical sanitizer of the appropriate concentration; . At time of inspection, wiping cloths were held in buckets at concentrations below required levels. CORRECTED, Sanitizer solution disposed of and new solution put in bucket at levels in compliance
3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection TCS foods were not being held at the proper temperature. At time of inspeciton, tossed salads inside cooler were measured at 47 degrees F for prolonged period of time. CORRECTED, salads and other food contents disposed of
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. At time of inspection, the Traulsen 4-door cooler appeared to not be in working condition as several internal reading tempertures were not in compliance
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Corrected During Inspection Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. At time of inspection, paper boxes and plates were observed on top of equpment near assembly line with food contact surface exposed. Please invert single use equipment to protect food contact surface CORRECTED, paper products put bottom up for storage

3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. At time of inspection, the condenser fan appeared to be in disrepair and ice was appearing throughout walk in freezer. Please repair fan and defrost to allow for surfaces to be cleaned.
The physical facilities shall be maintained in good repair.
