Critical Repeat Corrected During Inspection PIC unable to demonstrate knowledge by having no critical violations. PIC was able to correct all critical violations during inspection.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed no signed employee illness agreement for employee during inspection. PIC had signed at time of inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Repeat Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed two buckets of chlorine sanitizing solution to have a concentration less than 50ppm. Ensure all sanitizing solutions have a concentration of 50-100ppm at all times. PIC had corrected at time of inspection.

3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Repeat Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed chorine dishwashing machine to have a concentration less than 50ppm. Facility stated they will be discontinuing unit until it can work properly and will be utilizing the 3 compartment sink. Facility is to contact EHSIT when machine is repaired. Repair the machine by reinspection7/10/2024.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed milk on the cart that was returned from the dining room to have an internal temperature of 44F-45F. Currently the facility is utilizing an ice bath for the drinks while serving. The facility is to discontinue this practice until milk can be stored in complete temperature control the entire service time. PIC corrected at time of inspection.

3717-1-07.1(M) / Medicines - restrictions and storage.
Corrected During Inspection Unnecessary medicines and/or improper labeling/storing of medicines. Observed aspirin being held in junction with foods for sale in a black tote on the prep table. Ensure all medications and personal items are kept in an area completely separated from foods for sale. PIC relocated at time of inspection.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Corrected During Inspection Presence of live rodents. Observed rodent droppings in the corner of the kitchen near the mixer and in the back dry storage area behind the door. Facility was able to produce the last 3 months worth of pest control reports. Facility into continue sending EHSIT the reports until October 2024. Facility is to contact pest control operator and thoroughly sanitize the area. PIC cleaned droppings during inspection. Correct by reinspection 7/10/2024.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not protected. Observed backdoor near the kitchen to have gaps on side and bottom of the door allowing for light to pass through, therefore rendering the areas exposed to potential pest harborage. Obtain new door sweep/seals by reinspection 7/10/2024.

3717-1-04.8(E)(2) / Clean equipment and utensils stored in a self-draining position and covered or inverted.
Corrected During Inspection Clean equipment and utensils not stored in a self-draining position and covered or inverted. Observed silverware in the storage room to be stored with the food contact surface up and exposed to sneeze, splash, or dust. Ensure clean equipment is covered at all times. PIC covered at time of inspection.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Corrected During Inspection [52] Improper storage of single-service and single-use articles.Observed coffee filters to be uncovered and sitting with prepackaged bags of coffee grounds, rendering both sides of the coffee filters (which is a food contact service) exposed to sneeze, splash, or dust. PIC had corrected at time of inspection.

3717-1-04.1(A) / Equipment and utensils - durability and strength.
Repeat Non-durable equipment observed. Observed warewashing machine unable to produce and maintain proper concentration of chlorine. Facility stated plans to have repaired. Until then, facility is to utilize the 3 compartment sink. Correct by reinspection 7/10/2024.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed gasket in the walk-in cooler to be torn, rendering it no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Corrected During Inspection Nonfood-contact surfaces not easily cleanable. Observed foam mats being used at the bottom of the drawers for utensils to sit on,rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. PIC removed at time of inspection.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed expanding foam in the walk-in cooler, rendering the surface no longer smooth, durable, non-absorbent or easily cleanable. Correct by next standard inspection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed ice on the bottom of the walk-in freezer door, rendering the surface no longer smooth, durable, non-absorbent, or easily cleanable. Clean by reinspection 7/10/2024.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Insufficient air gap between the flood rim and the water supply inlet. Observed insufficient air gap under the prep sink (1/2 inch). Per Ohio Uniform Food Code, "An air gap... shall be at least twice the diameter of the water supply inlet and may not be less than one inch". PIC had corrected during inspection.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed the hot handwashing sink in the kitchen to have issues turning on and staying on. There is enough velocity and heat to effective wash hands, however the sink will abruptly shut off at time. maintenance was on site attempting to fix the issue. Correct by reinspection 7/10/2024.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed missing baseboard under the warewashing sink, and a whole in the ceiling above the warewashing sink. Correct by next standard inspection.

3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. Observed dry wall over the warewashing area and in the dry storage area, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed visible residue on the floors of the back dry good storage area. Clean by reinspection 7/10/2024.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed cracked and missing tiles throughout the facility, rendering the surfaces no longer smooth, durable, non-absorbent, or easily cleanable. Correct by next standard inspection.

Comments
Conducted and reviewed with PIC, maintenance, and EHSIT.
Facility's risk level 4 activities include: cook/cool/reheat and serving a HSP.
Due to repeat corrected critical violations for item number 45. A supervisor will be accompanying EHSIT on reinspection.
Reinspection to occur on 7/10/2024.
